New patients are asked to print the Patient Packet, read all materials, print and complete the forms, and return the completed forms to Albany Health Management Associates.
The Patient Packet consists of:

- Scheduling and Payment Policies
- Notice of Privacy Practices
- Client Intake Form*
- Approval for Substitute Signature*
- Release of Information Authorization Form*
- Acknowledgement and Receipt of Notices*
